Third Party Fundraising Registration Form

To ensure a positive experience for all parties involved, the following guidelines are essential for a successful partnership:

1. Surrey Hospitals Foundation should review and approve all sponsors and promotional materials prior to distribution. Permission through the Foundation’s Chief Operating Officer is required in order to use the name of Surrey Memorial Hospital, Surrey Hospitals Foundation, Jim Pattison Outpatient Care and Surgery Centre and logo(s) for publicity and fundraising purposes by community groups and individuals.

2. Since Surrey Hospitals Foundation may be the recipient of funds from its participation in other similar events, it is understood that this agreement creates no exclusive rights to the Third Party for this event.

3. All staffing and volunteers for the special event/promotional campaign will be provided by the Third Party unless otherwise agreed in writing.

4. All monetary transactions for the special event/promotional campaign will be handled by the Third Party (i.e. ticket sales).

5. Surrey Hospitals Foundation will incur no financial costs associated with the Event. All financial liabilities will be assumed by the Third Party. Neither Surrey Hospitals Foundation or its employees or volunteers will be liable for any injury or death of any participant, customer or workman employed in connection with the conduct of the Event or the care and maintenance of any equipment used in the event, or the surrounding grounds, buildings, and facilities or any other employees of nay place where the Event shall be conducted nor shall Surrey Hospitals Foundation or its employees or volunteers be liable for any property damage which may occur during the term of the event covered herein.

6. Surrey Hospitals Foundation does not provide access, loan or give out its mailing list for any special event/promotional campaign.

7. As a guideline, all charitable events should net minimally 50% of the gross income, to be applied as a donation. Surrey Hospitals Foundation is required by law to demonstrate a reasonable allowance in the case an excessive percentage of net proceeds being allocated to non-charitable expenses.

8. Surrey Hospitals Foundation reserves the right to inspect the financial records of the Third Party regarding funds collected as the result of the event

9. The proceeds of the special event will be presented to Surrey Hospitals Foundation in a timely manner following the special event unless otherwise agreed in writing.

10. Recognition for the Third Party donation to Surrey Hospitals Foundation will be in accordance with the Foundation’s established Donor Recognition Guidelines.

11. Surrey Hospitals Foundation will not partner with nor support events that discriminate against any person or group based on gender, race, class, economic status, ethnic background, sexual orientation, age, physical ability, and cultural and religious backgrounds. We reserve the right to decline events that promote the use of alcohol or tobacco; endorses a political party or candidate or appears to endorse a political agenda.

12. All Surrey Hospitals Foundation approvals and authorizations are to be secured through a verified Foundation representative.

13. This agreement constitutes the sole agreement between the parties and no amendment, modification, or waiver of any terms and conditions shall be valid unless in writing.
